Thank you for looking to enter the 2024 Business disABILITY Awards. Please complete all sections of this form before submitting your entry.  We recommend you consider the below application tips before completing your application.  
If you cannot complete the entry form via the following online form, you can submit your entry via other means including, audio recording, video recording, document submission. Complete your written or video submission and email the committee here. If you are completing a video entry compared to a written entry, we recommend you use WeTransfer to upload and send your self-recorded video file.

Application Tips 

  • Review each of the award categories carefully and select the best category for your contribution or circumstance. You can enter more than one award category but can only win once. 

  • Check you are eligible to enter the category of your choice. 

  • Review all award entry questions before starting your application. 

  • Carefully consider how you meet each of the award criteria for your category. 

  • If you require assistance to complete your entry including if it is video entry, ask a friend, family member, mentor or support person for help. Please note that if you are an individual, we can arrange for you to attend a video interview where you can be supported by a carer/support person.  If you would like to arrange a video interview, please request this by emailing the committee here. 

  • Answer all questions related to your chosen award category - be specific and include as much detail as you can, examples, results and measurements to support your entry. 

  • You are encouraged to include quality photos and a supporting document (e.g. testimonial) that will help tell the story of your contribution or project. You can upload supporting documents at the end of the entry form or email supporting assets to the committee here.

Please note, as part of the entry process, companies and individuals are required to provide at least one reference for the judging committee to contact with.
